The Symptoms and Perceptions (SaP) questionnaire is a reliable and valid open-access instrument designed for the epidemiological examination of physical and psychological symptoms, including health complaints following disasters. With a foundation in primary care symptom classification, the SaP assesses a broad range of symptoms across multiple organ systems by evaluating their occurrence, duration, and perceived severity. Additionally, it captures perceptions, such as symptom beliefs and causal attribution, to gain more insight into the subjective impact and potential causes of the symptoms reported.

This paper presents the development rationale for the SaP, provides a comprehensive overview of its key features, and argues for its importance as a valuable tool for symptom assessment and monitoring.
