De syndroomdiagnose ‘prikkelbaredarmsyndroom’ (PDS) wordt per exclusionem gesteld, maar het is de vraag hoeveel diagnostiek er plaats moet vinden om met een redelijke mate van zekerheid de diagnose te kunnen stellen. Wij presenteren op basis van een systematische review de diagnostische waarde voor het uitsluiten van PDS op basis van de verschillende PDS-criteria. Het vermogen van de verschillende criteria om PDS te onderscheiden van organische aandoeningen is erg variabel en valt tegen. Patiënten die aan die criteria voldoen blijken een lagere kans op een organische aandoening te hebben dan patiënten met buikklachten die daar niet aan voldoen. Dat geldt ook voor de diagnostische waarde van afzonderlijke alarmsymptomen. Die blijken frequent voor te komen bij PDS patiënten zonder dat er sprake is van een onderliggende organische darmaandoening. Een organische aandoening kan niet goed worden uitgesloten op basis van symptoomcriteria. Maar de lage priorkans op een organische aandoening bij huisartspatiënten die voldoen aan PDS-criteria pleit tegen het verrichten van uitputtend diagnostisch onderzoek. (aut.ref.)
The syndrome diagnosis 'irritable bowel syndrome' (IBS) is often made on the basis of exclusion, but the question is how many diagnostic tests should be performed in order to establish this diagnosis with a degree of confidence. We present the diagnostic value of various IBS criteria for excluding IBS, based on a systematic review. The potential of the various criteria for distinguishing IBS from organic disease is extremely variable and disappointing. Patients fulfilling IBS criteria have, however, a lower risk of organic disease than patients with abdominal symptoms who do not fulfil the criteria. The same holds true for the diagnostic performance of individual alerting symptoms. These seem to be present frequently in IBS patients in whom there is no underlying organic bowel condition. An organic condition cannot be accurately excluded on the basis of symptom criteria. However, the low prior risk of organic conditions among patients who consult a primary care doctor and who meet IBS criteria argues against exhaustive diagnostic evaluation.
